All high school track participants must submit the appropriate consent forms. Consent forms are required from all high school track participants including instructors, advisors, students, and others. You can download the necessary consent forms below. Print and complete these forms, then mail them to the address at the bottom of this page. Please also see the Calendar of Events for the deadline for team rosters.


If your team is using high school laboratory facilities, the team will need to submit the following consent forms:

  1. Each high school student must submit a participant consent form signed by his/her parent or guardian
  2. Each instructor must submit a participant consent form
  3. Each team advisor/mentor must submit a participant consent form
  4. A principal of each high school represented on the team must sign the principal consent form


If your team is using laboratory facilities outside of the high school, the team will need to submit the following consent forms:
